Abstract
This disclosure describes a method of making incident reporting a condition of holding a live certification mark for AI systems. The certified system's own tamper-evident ledger is watched for declared trigger events, such as an activation of its independent reset in live operation, a lapse of its power lease outside a drill, the governor reaching its hold level, a breach of data custody, or a security compromise. When a trigger is detected, an initial notice is generated automatically from the ledger and sent through an authenticated channel to the independent certification body, so that reporting does not depend on the operator choosing to report. The operator must follow with a full report giving the cause and fix within a declared period. Confidential details stay with the certification body; a plain-language summary is published in a public registry, linked to the system's certification record. Near misses may be reported voluntarily with protection from penalty. Honest reporting does not by itself affect the mark; failure to report a detected trigger, or a mismatch between the ledger and the report, leads to suspension. Trends are analysed and published so that every operator learns from every incident.
