Abstract
A method and system for showing a clinician, alongside an open consultation and without extra work, where each item being recorded (a coded diagnosis, symptom, procedure, measurement, medicine or short note) has appeared before in the same patient’s record. A program on the clinician’s workstation notices entries as they are saved, by observing the electronic health record’s own traffic or interfaces and re-reading the consultation draft, and binds every result to one patient by requiring several independent sources to agree and discarding late answers. It retrieves the patient’s history through the record’s own interfaces, matching by concept identifier, ancestor concepts, problem-list links and wording, and keeps all data in memory on the workstation. The history is shown as a sized term cloud and, for each term, a month-by-year calendar, last and earlier actions, outstanding items, measurement graphs, referral-to-letter matching and mentions in notes. Terms linked to the problem list are shown apart from acute presentations. Those are grouped into separate episodes, with a seasonal marker and older history marked rather than cut off. Optional, labelled demonstration layers match records against published recurring-presentation patterns and combine published single-feature figures into illustrative scores, with the condition hidden until requested. Variations cover browser and desktop records, standard interfaces and AI-assisted reading of free text within the record’s boundary.
